PhysioCare & Rehab (“the Clinic”) is committed to safeguarding personal and personal health information in accordance with Ontario’s Personal Health Information Protection Act (PHIPA), the Personal Information Protection and Electronic Documents Act (PIPEDA), Canada’s Anti-Spam Legislation (CASL), and the standards and record-keeping requirements of the College of Physiotherapists of Ontario, the College of Chiropractors of Ontario, and the College of Massage Therapists of Ontario. The Clinic collects, uses, and retains only the information reasonably necessary to provide physiotherapy, chiropractic, massage therapy, and rehabilitation services, including contact information, health history, assessment findings, treatment records, billing details, and insurer or third-party documentation. Information is collected directly from patients or with lawful consent and is used strictly for clinical care, administrative purposes, quality assurance, risk management, regulatory compliance, lawful reporting obligations, and insurance processing. Personal health information may be disclosed to insurers, referring healthcare providers, regulatory authorities, or legal bodies where authorized by the patient or required by law. Records are stored using appropriate administrative, physical, and technological safeguards with restricted access and are retained in accordance with applicable professional College retention requirements. Patients may request access to or correction of their records in writing, subject to legal limitations and reasonable administrative fees.
By voluntarily seeking assessment or treatment at the Clinic, patients provide informed consent to evaluation, therapeutic intervention, clinical documentation, and the collection, use, and disclosure of personal health information for the purposes described above. Patients acknowledge that health outcomes cannot be guaranteed and that treatment recommendations are based on professional clinical judgment within regulated scopes of practice. Patients further acknowledge that electronic communications, including email and website submissions, carry inherent privacy and cybersecurity risks beyond the Clinic’s reasonable control.
The Clinic does not sell, rent, or trade personal information. Any electronic communication sent by the Clinic, including appointment reminders, clinic updates, or educational materials, is provided in compliance with CASL; recipients may withdraw consent to receive non-essential electronic communications at any time using the unsubscribe mechanism or by contacting the Clinic directly. Essential clinical and administrative communications related to ongoing care may continue as permitted by law.
